Registering With Us  

How to register

 

We take any eligible NHS patients from within our practice area (see below).

 

We will ask you to complete a GMS1 form and our own new patient form (see below). If you do not have your NHS number then we will need the full details of the last address at which you were registered with a GP along with details of your previous GP. To prevent fraud we also require proof of your address.  There are exceptions to this, please ask to speak to the Practice Manager.

Please pick up the new patient form from reception. It is helpful to have your medical card with you as this will speed up the process of getting your medical records from your previous GP Practice. If you do not have a medial card then we will need the full details of the last address at which you were registered with a GP along with details of your previous GP.

 

Practice Area

 

We accept patients living in the area bordered by the Western Avenue (to the north), Great West Road (to the south), Northfields Avenue/Argyle Road (to the west) and Acton Underground line (to the east).

 

We work closely with a small number of neighbouring practices and feel that there would be little advantage from moving to us from either Crown Street or Mill Hill Surgery. Individuals may discuss this with any of the doctors. Following changes to the NHS, registration is now with the Practice rather than an individual doctor, but on registration preference to see a particular Doctor can be recorded and respected when possible, in the case of an emergency this may not always be possible.

 

If you move out of the area you will be asked to find a new GP as we cannot then visit in cases of emergency.

 

Named GP

All of our patients are allocated a named GP.  For the ease of patients and to ensure an even distribution across our GPs this is allocated according to patient surname.

 

Dr Ali: surname begins with A or B

Dr Chu (covered by Dr Pojak): surname begins with C, D, E, F or G 

Dr Dhatt: surname begins with H, I, J or K

Dr Mirza: surname begins with L, M, N or O

Dr Shiryaeva: surname begins with P or S

Dr Tailor: surname begins with Q, R, T, U, V, W, X or Y or Z

 

Hillcrest Surery is a group practice with care provided by a dedicated team of GPs.  Patients are free to see or book a telephone consultation with any GP (except in emergencies when they may need to see the Duty Doctor).  The named GP is largely a role of oversight.
